Job Description SummaryThe Performance Improvement/Quality Liaison/RN participates in the various activities of performance and quality improvement, orientation, utilization, reimbursement and documentation.
The Performance Improvement Liaison plays a key role to assure effective utilization of the Home Care Division's resources in order to maintain the highest quality of care achievable.
The ideal candidate will be an RN with demonstrated abilities working with the Joint Commission, performance improvement initiatives and quality/sustainable solutions within fiscal responsibility.
This is a hybrid position.
Job DescriptionKey
Responsibilities:
o On an ongoing basis, acts as a resource/team liaison for the Home Care Division/VNA.
o Provides ICD-10 coding excellence using specialty knowledge to educate team members, clinical managers and clinicians.
Provides individual and team/department education based on identified needs.
o Performs required joint visits with team members to determine accuracy of assessments, OASIS documentation, integrating reliability, and other areas per joint visit tool.
o Educates staff regarding their role in improving patient outcomes and appropriate coding using ICD 10 coding guidelines.
o Reviews, evaluates and interprets data related to documentation from internal and external sources to promote documentation accuracyoIncorporates ICD-10 coding principles to assure the diagnosis codes accurately reflect the patient's condition and plan of care requirements.
o Assists the Staff Educator and Training Coordinator to provide and coordinate the Home Care Division (HCD) New Hire Orientation.
oActs as OASIS software administrator, assures accuracy of the process and maintains compliance with State and Federal Regulations through correct encoding, locking and transmission of the data to the Department of Public Health and any other appropriate vendors.
oDevelops and continuously expands in depth knowledge of insurance coverage criteria as evidenced by correspondence from/with fiscal intermediary and/or HMO/private insurance payors.
o Participates in state and regional homecare meetings.
oChampions a Joint Commission standard chapter as assigned.
oChairs at least one Home Care Division (HCD) committee/subcommittee as evidenced by meeting minutes.
o Demonstrates clinical proficiency and competency in the electronic medical record as evidenced by successful completion of education program and appropriate daily utilization of software.
o Utilizes information systems and telecommunication services as instructed, this includes phones, oFosters a Culture of Safety through personal ownership and commitment to a safe environment.
Knowledge and Abilities:
oKnowledge of PI process and process evaluation techniques (FMEA)oDemonstrated ability to interpret qualitative and quantitative data.

Bachelor of Science Degree in Nursing from an NLN approved school of nursing.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
