Day (United States of America) Under the direction of the Director Administrative Operations, assist in the coordination of the marketing and intake services for the Home Care Division.

Assist in the overall operations of the Admissions/intake department.
b.
Understands daily flow of department and delegates work appropriately and fairly as directed by team leader and/or director c.
Ability to prioritize daily job functions to meet the needs of the department and organization.
d.
Creates a daily admissions roster with each expected admission and updates it through out the day.
e.
Works with community liaisons and referral sources to follow up on potential discharges as need.
f.
Communicates effectively with department, managers, clinicians, and referrers.
g.
Manages all electronic forms of communication including, faxes, voice mail, E-fax, electronic medical record and electronic referral platforms to ensure consistent and reliable flow of referral information.
h.
Attends all department meetings and actively participates in the structure and processes of the department.
i.
Recommends modifications and assists with improvements in department systems and policies.
2.
Ensures the appropriate completion and flow of documentation following Department policies as designated by the team leader and/or manager a.
Reviews referral information to ensure accurate and timely information received and follow up as appropriate.
b.
Ensures appropriate flow and completion of referral information for dissemination to all managers, disciplines and ancillary departments.
c.
Maintains projected admissions report for use by managers in a timely manner from information received from admissions department including community liaisons and intake staff.
d.
Confirms Plan of Care physician for referrals.
e.
Removes patient referral when referral source notifies agency of cancellation.
f.
Enters referral demographics into appropriate application.
g.
Able to access 485/Recertification Meditech screens to identify incoming referral status as admissions or resumption of care 3.
Demonstrates strong communication skills and positive customer service.
a.
Promotes positive Agency Image with professional courteous telephone manner at all times.
b.
Demonstrates telephone skills that are efficient, helpful and concise c.
Uses automated Call Distribution Phone System correctly, monitoring calls waiting in Que and picking up all messages in the group voice mail box.
d.
Communication is effective, clear and collaborative.
e.
Uses health system communications resources to inform internal and external audiences of departmental news/achievements/policies.
4.
Performs general clerical duties a.
Maintains adequate office supplies for the department.
b.
Takes meeting minutes as requested and types meeting minutes, guidelines, and procedures as requested.
c.
Consistently arrives to workstation on time-prepared to answer phones.
d.
Maintains a high level of accuracy, legibility, thoroughness and neatness in written, typing and word processing duties.
5.
Technology-Embraces technological solutions to work processes and practices.
a.
Demonstrates proficiency and competency in assigned electronic medical record modules b.
Utilizes information systems, telecommunication service and office machines as instructed, this includes phones, email, paging systems, fax machines and copiers.

Demonstrates proficiency in the assigned agency software and the Microsoft office suite of products 6.
